What are the responsibilities and job description for the Medication Management RN Health Coach position at Astera Health?
This position is scheduled for 48 hours per biweekly pay period, Monday - Friday, rotating days. Responsibilities required, but not limited to:
- Serves as a patient advocate.
- Assess the client’s emotional status, cognitive status and learning readiness within the client’s environment to augment a holistic plan of care.
- Facilitates mutual goal setting and goal directed interactions with the client/family regarding specific disease processes.
- Identifies patients not meeting clinical goals and arranges follow-up services by protocol or as appropriate.
- Works with patients and families on self-management support. Assumes responsibility and accountability for care plan effectiveness and client outcomes.
- Monitors, evaluates, and notates trends in patient responses and identifies risk issues, variances, and improvement opportunities.
- Enters and submits data accurately and in a timely manner.
- Maintains cost-effective care by monitoring proper utilization of interventions, supplies, and equipment.
- Enhance patient’s confidence enabling them to carry out a behavior or skill necessary to reach a desired goal. Enable people to be in more control of their health and engage them in their disease management.
- Provides physicians with organized office visits and makes the best of their time with the patient.
- Track and manage patients proactively and assist the physician in providing opportunities for care.
- Develop relationships through interaction with health care providers in diverse care settings to ensure the reliability of patient coordination between all clinicians.
- Assist with prescription refill documentation.
- Current licensure as a Registered Nurse (RN) in the state of Minnesota.
- 2 years of experience in case management; or combined years of relevant education and experience to perform the essential duties of this position.
