SIS Assessor
As an Assessor at Astyra, you will be responsible for face-to-face assessments of individuals with intellectual and developmental disabilities. Persons being assessed reside in the community or in facilities. You may also complete review decisions regarding services needed to be successful in the community and will conduct training of new assessors and quality assurance competency reviews as assigned.
We are seeking candidates available to work in the Central/Shenandoah Valley Region of Virginia. Areas could include: Charlottesville, Staunton, and Harrisonburg.
WHAT IS THE SIS?
Support Intensity Scale Assessors schedule and complete a standardized assessment with adults that have a developmental or intellectual disability. The resulting report is utilized by providing agencies and caseworkers to advocate and enhance the Person Centered Planning Process. A typical SIS takes from 1.5 to 2.5 hours to complete and is a guided conversation concerning the individuals unique support needs.
WHAT ARE THE REQUIREMENTS?
- Bachelor’s Degree in human services, healthcare, or social work
- Minimum of 3 years working with individuals who have disabilities.
- Ability to travel as part of the SIS assessor team
- A commitment to longevity in the developmental disabilities system and the SIS program
- Excellent communications skills and computer
- Must successfully pass SIS Certifications for adults and children upon hire.
WHAT ARE ASSESSORS EXPECTED TO DO?
- Complete SIS training programs including observations, guided practices, classroom work and Interviewer Reliability and Qualification Review Assessments
- Comply with all applicable regulations and requirements (HIPAA, Medicaid, etc.)
- Record data in SIS online databases
- Understand and be able to explain SIS results
- Attend regular SIS team meetings
