We are seeking a passionate and dedicated Home Health Occupational Therapist to join our team. As part of the BrightSpring Health Services family of brands, AT Home Care & Hospice is committed to providing exceptional patient care.

Responsibilities:

• Assesses and analyzes patient care needs and establishes mutual goals with the patient and/or caregiver for a comprehensive plan of care.
• Assesses the patient support system and utilization of available community resources to ensure optimum level of care.
• Identifies therapy diagnosis/patient care problems and interventions; makes additions or changes to ensure the patient problems are comprehensively addressed and progress documented.
• Documents additions and/or changes to the patient plan of care.
• Re-evaluates the patient's progress and therapy program making changes as necessary under the physician's direction.

Requirements:

• Graduate of an Occupational Therapist Education program approved by ACOTE.
• Two (2) years of experience in adult occupational therapy, home health experience preferred.
• Occupational therapy skills per competency checklist.
