What are the responsibilities and job description for the RN Field Supervisor position at At Home Care of Kentucky?
This job oversees nursing services for our private duty patients, both pediatric & adults. This position will involve travel for new patient admissions, recertifications, supervisory visits, etc. This position reports to the Director of Patient Care Services.
The following duties are normal for this position. Other duties may be required and assigned.
- Obtain initial comprehensive assessments and patient admissions.
- Provide post hospitalization and ongoing re-assessments.
- Provide patient, family, and nursing education.
- Develop and update the patient's care plan.
- Ensures LPN/RN supervision and education through performance feedback and coaching as well as regular supervisory home visits, proficiency evaluations, and competency evaluations to be completed at required intervals.
- Regularly visits patient homes to observe caregiver interactions, monitor patient well-being, address any concerns raised by patients or caregivers, and ensure adherence to care plans.
- Ensures compliance with all applicable company, insurance, state, and federal rules and regulations.
- Obtain interim medical orders, as needed.
- Must be able to maintain accurate and timely documentation.
- Communicate with the care team and physicians.
- Completes work with accuracy and within time frames.
- Notifies the Director of Patient Care Services of any anticipated problems with the posted schedule. Provides as much advance notice of changes/problems as is possible.
- Adheres to agency infection control and safety policies, including education, reporting and practice implementation specific to job position.
- Keeps Director of Patient Care Services informed of events, problems, and concerns at all times.
- Maintains current/valid Kentucky nursing license.
- Maintains confidentiality of all records and reports maintained for all patients.
1. Must be eighteen years of age or older.
2. Must have completed an accredited LPN or RN nursing program and hold a current license to practice in Tennessee.
3. A thorough knowledge of the framework, organization, and function of the Personal Support Services program strongly preferred. Training may be possible for the right candidate.
4. Must have a thorough knowledge of the principles and methods involved in supervising field staff (Personal Care Attendants and CNAs).
5. Must be capable of performing job duties physically, mentally, and independently.
6. Excellent communication and interpersonal skills.
7. Excellent organizational and time management skills.
8. Ability to work well work independently and in groups.
9. Must have reliable transportation. The work requires the ability to safely transport oneself to and from multiple client homes and work sites on a regular, daily basis in a multi-county area.
10. The work will require some lifting, pulling, bending, pushing, stooping, kneeling, reaching, and carrying of light and heavy items, such as books, papers, files and/or office supplies, often at least 20 lbs. weight.
11. There will be short to long periods of transferring, standing, and sitting.
12. Must have mental ability to exercise good judgment in decision-making, including while driving and while in client homes.
13. Ability to walk independently up and down a normal flight of 13-15 stairs.
