Home Healthcare Specialist

Key Responsibilities

• Evaluate patients with physical, psycho-social and/or cognitive impairments to determine if occupational therapy can benefit the patient's quality of life.
• Plan, implement, and evaluate patient care plans in conjunction with the physician and care team to restore or maintain patient well-being.
• Document care provided and participate in the coordination of care with other care team members.
• Provide education and training to patient caregivers and family.
• Supervise Certified Occupational Therapist Assistants and Aides, when applicable, in the performance of patient care that is consistent with the patient's needs.
