Hospice Nurse Practitioner (NP) – Ft. Worth, TX
Join our compassionate hospice team in Ft. Worth, TX, as a Nurse Practitioner (NP) providing essential support and care to our patients and their families. We are seeking a skilled NP to conduct face-to-face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.
Key Responsibilities:
- Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support their clinical needs
- Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
- Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
- Document patient encounters accurately and timely to ensure regulatory compliance
- Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources
Qualifications:
- Education: Master’s degree or Doctorate in Nursing, with current NP licensure in Texas
- Experience: Minimum of 1 year of experience as an NP in a hospice or palliative care setting
- Certification: Current Advanced Practice Registered Nurse (APRN) certification; CPR certification
- Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care
- Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
