Position Summary:
The Care Manager will also function as the vocational specialist. This position will assist consumers with linking to, and accessing, services and supports described by the consumer as essential in realizing their goals. This position will provide an array of services to assist consumers with re-entering the workforce or educational programs. The Care Manager will support consumers in transitioning into, and maintaining, their tenure in the community. The Care Manager will begin engaging consumers while they are still residing in state psychiatric hospitals and work with them on developing a WRAP. The Care Manager will also coach consumers the various physical, cognitive/intellectual, and behavioral skills related to the consumer’s identified goals in a focused manner that leads to increase competency and proficiency. This could include daily living skills, socialization skills, and skills in accessing medical treatment, skills in accessing appropriate entitlement programs, skills related to recreational activities, and communication skills. .
Duties and Responsibilities:
• Engaging with program participants in a warm, empowering manner.
• Exhibiting a person-first approach in all interactions and interventions.
• Providing skills-building education.
• Assisting individuals in developing Wellness and Recovery Action Plans and Psychiatric Advance Directives.
• Participating, with other team members and consumers/family/others as appropriate, in the completion and updating of treatment plans for each consumer within established time frames or as indicated by consumer need.
• Explaining to consumers their rights and procedures for removal/discharge from housing.
• Providing individual and family therapy and supportive counseling.
• Providing crisis mitigation interventions.
• Completing a comprehensive assessment.
• Initiating linkage with community services.
• Providing interventions to stabilize the community living situation.
• Performing outreach, engagement, and assessment to enroll consumers into the program
• As part of the team, assisting consumers in selecting housing
• Providing 24/7 on-call support for consumers.
• Maintaining program compliance to DMHAS contract requirements.
• Meeting or exceeding contracted commitments including budgeted volume of service.
• Maintaining valid NJ driver’s license.
• Providing individual supportive counseling to assist consumer to identify and cope with the symptoms of mental illness that may interfere with his/her work performance.
• Providing on the job crises intervention
• Providing work-related supportive services (assistance with grooming and personal hygiene, securing appropriate clothing, wake -up calls, and transportation.
• Completing an assessment of consumer work skills
• Making recommendations to the treatment plan regarding vocational goals
• Participating, with other team members and consumers/family/others as appropriate, in the completion and updating of treatment plans for each consumer within established time frames or as indicated by consumer need.
• Responding to consumer defined crises 24 hrs per day, 365 days per year, providing crisis intervention and consultation to consumers, their families and others
Licensure/Certification: Current and valid NJ driver’s license
Minimal Qualifications:
Education: Bachelor’s degree with a major in a mental health or other human services discipline
Experience: A least one year experience providing care management and community support for individuals with serious mental illness, including assisting consumers with their vocational needs
