Job Summary:
The Community Health Worker works closely with primary care providers and/ or addiction medicine providers to facilitate referrals for counseling, provides case management – assisting emergency department patients to identify and access the help they need. Document encounters in the electronic medical record ensuring that all pertinent information including; referrals, release of information, consents and other clinical documentation is entered appropriately. Cross-training between primary care provider locations: Tully Family, Athol Hospital, & Athol ED.

Essential Functions

- Maintains a clean and safe hospital.

- Maintains a safe and healthy environment for patients, visitors, and/or staff to provide the highest level of physical comfort, and minimize risk and injury.
- Ensures compliance with regulations to maintain accreditation and licensure.
- Complies with the Hospital Attendance and Tardiness Policy
- Completed all Hospital and Department Specific Mandatory requirements in the prior calendar year.
- Reports Incidents in a timely and effective manner.
- All employees with direct patient contact are responsible for actively engaging in the practice of, monitoring and enforcing compliance to hand hygiene.
- Managing assigned caseload of patients, maintaining accurate records, attending meetings and providing weekly reports.
- Coach patients in effective management of their chronic health conditions and self-care.
- Assist patients in understanding care plans and instructions. Provide culturally and language appropriate services.
- Help patients in properly accessing and utilizing resources including scheduling appointments, assisting patients with completing applications and registration forms.
- Assist patients in accessing health related services and overcoming barriers to obtaining needed medical care and social services.
- Facilitate communication between patients, services and providers. Encourage patients to be active, engaged participants in their health.
- Help patients connect with transportation resources and give appointment reminders in special circumstances.
- This includes a strong role as patient advocate.
- Assist emergency department patients in accessing tele behavioral counseling services.
- Conduct screenings and assessments using various tools to determine mental health status and social needs.
- Coordinate, manage and schedule tele behavioral counseling sessions.
- Manage data and appointments in hospital’s electronic medical record.
- Obtain insurance authorizations for clinical sessions.
- Provide care coordination to emergency department patients.
- Provide ongoing follow-up, motivational interviewing and goal setting with patients.
- Continuously expand knowledge and understanding of community resources and services.
- Collaborate as a member of the emergency department team to discuss patients on the caseload.
- Participate in weekly supervision and program staff meetings.
- Track and submit data; complete monthly reports.
- Manage time and resources in a skilled manner.
- Understand health care culture and work closely with school personnel.
- Demonstrate appropriate boundaries in all interactions.
- Attend relevant trainings, conferences, and community events.
- Follow Heywood Healthcare’s policies and procedures.
- Maintain confidential information.
- Additional duties will be at the discretion of the department manager.

Qualifications
Job Requirements
Minimum Education
- Associates Degree required, or 2 years of experience in lieu of AD.
- Community Health Worker certification is required to be completed within 6 months of the hire date. Funding provided for CHW certification requires 1 year of fulfillment of the RCORP role.
Minimum Certifications
- AHA BLS required within 90 days of hire

- Strong interpersonal and communication (written and oral) skills
- Strong computer skills – electronic medical record databases, Google calendar, Zoom
- Knowledge of area community resources and agencies
- Skill in organizing resources and establishing priorities
