We are actively searching for an AGHS RN Care Coordinator. The Care Coordinator’s primary responsibility is to oversee care coordination of activities for the patients in certain physician practices. The care coordinator offers a broad set of longer term services that include medical coordination, and assistance in navigating the system, with both quality enhancement and cost reduction goals. This will include developing and monitoring care coordination processes and support clinical teams with these efforts. It will also include identifying the high acuity patient population and working more directly to ensure care management for this patient population. The position may involve some patient triage. The Care Coordinator will work with the Clinical Manager; Medical Director and Practice providers to develop this position to best serve the needs of the patient panel and the primary care teams.
The ideal candidate will have a current RN license, with a BSN or related Bachelor's Degree preferred. Candidate must have five years of clinical experience, including experience in the physician’s practice and case management. Must demonstrate knowledge of the Patient Centered Medical Home primary care delivery model. Experience with E.H.R. is preferred. Must have knowledge of care plan development, community patient resources and have the ability to work in a fast-paced environment balancing multiple demands. Must have proficiency with Microsoft Office computer applications, be highly organized and possess excellent written and verbal communication skills.

Salary Range: $32.00/hour to $56.00/hour (credit given for relevant experience)
