Responsibilities
- Conduct comprehensive assessments of clients' needs and strengths.
- Develop, implement, and monitor individualized service plans.
- Provide crisis intervention and support to clients facing immediate challenges.
- Facilitate access to community resources, including healthcare, housing, and workforce development services.
- Collaborate with multidisciplinary teams to ensure holistic care for clients.
- Maintain accurate documentation of client interactions and progress in compliance with agency policies.
- Utilize motivational interviewing techniques to empower clients in their decision-making processes.
- Supervise support staff as needed, ensuring quality service delivery.
- Stay informed about relevant programs such as Section 8 housing assistance and Medicare services.
Qualifications
- Bachelor’s degree in Social Work or a related field; Master’s degree preferred.
- Experience in case management or social services is highly desirable.
- Knowledge of child welfare systems and childcare practices is a plus.
- Familiarity with addiction counseling techniques is beneficial.
- Strong skills in crisis intervention and conflict resolution.
- Excellent communication skills, both verbal and written.
- Ability to work independently as well as part of a team.
- Proficiency in using case management software and other relevant technology.
