Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients in various settings, including home care, primary care, and within a Level II Trauma Center. The LPN will play a vital role in patient assessment, triage, discharge planning, and education, ensuring that patients receive the highest quality of care.
Responsibilities
- Deliver direct patient care under the supervision of registered nurses or physicians.
- Conduct patient assessments and document vital signs accurately.
- Perform phlebotomy procedures as needed for laboratory testing.
- Assist in triaging patients to prioritize care based on urgency.
- Participate in discharge planning to ensure smooth transitions for patients leaving the facility.
- Educate patients and their families on health management and treatment plans.
- Collaborate with interdisciplinary teams within a teaching hospital environment.
- Utilize ICD-10 coding for accurate documentation and billing processes.
- Provide care in pediatrics as required, ensuring comfort and safety for younger patients.
Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of HI
- Experience in home care or primary care settings is preferred.
- Familiarity with working in a Level II Trauma Center is advantageous.
- Proficiency in ICD-10 coding systems is a plus.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to work collaboratively in a fast-paced environment while maintaining attention to detail.
