Responsibilities
- Assess patient needs and develop comprehensive care plans that address medical, psychological, and social factors.
- Coordinate patient care across multiple disciplines, including hospital medicine, pediatrics, and infectious disease care.
- Monitor patient progress through regular evaluations of vital signs and overall health status.
- Facilitate discharge planning to ensure smooth transitions from hospital to home or other care settings.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
- Engage in utilization management to optimize resource use while ensuring quality patient outcomes.
- Collaborate with multidisciplinary teams to enhance patient care and address complex health issues.
- Provide education and support to patients and their families regarding treatment options and care plans.
Skills
- Proficient in managing medical records and documentation processes.
- Strong knowledge of utilization management principles.
- Experience working in a Level II Trauma Center is preferred.
- Background in hospital medicine or related fields is essential.
- Familiarity with infectious disease care protocols.
