Job Summary
We are seeking a dedicated and detail-oriented Case Planner to join our healthcare team. The Case Planner will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their treatment journey. This position requires a strong understanding of patient needs, effective communication skills, and the ability to collaborate with various healthcare professionals.
Responsibilities
- Develop and implement individualized care plans for patients, focusing on their specific needs and goals.
- Coordinate with healthcare providers to ensure comprehensive patient care, including pediatrics and primary care.
- Facilitate discharge planning by assessing patient readiness and arranging necessary follow-up services.
- Conduct assessments related to esthetic laser treatment, outpatient services, and other specialized care as needed.
- Oversee IV insertion, catheter care, and specimen collection processing as part of the patient management process.
- Maintain accurate documentation of patient interactions and care plans in compliance with healthcare regulations.
- Collaborate with multidisciplinary teams to enhance patient outcomes and streamline service delivery.
Experience
- Previous experience in case planning or a related field is preferred.
- Familiarity with pediatrics, primary care, esthetic laser treatment, outpatient services, discharge planning, IV insertion, mammography, catheter care, and specimen collection processing is highly desirable.
- Strong organizational skills with the ability to manage multiple cases simultaneously.
- Excellent communication skills for effective interaction with patients, families, and healthcare professionals.
