Position Summary:
The Referral Specialist is responsible for supporting the continuity of care for clients by ensuring timely and accurate collection of necessary documentation from families and providers. This role involves coordinating with both internal teams and external providers to facilitate the seamless processing of continued care and authorizations.
Key Responsibilities:
- Document Collection:
- Communicate with families and/or medical providers to gather required documentation for service continuity.
- Ensure all collected documents are complete and accurate per insurance requirement guidelines.
- Provider Outreach:
- Contact healthcare providers to request updated documentation and signatures on necessary forms.
- Follow up with providers to ensure timely receipt of requested documents by authorizations deadline.
- Collaboration:
- Work closely with internal departments and teams to identify and obtain needed documentation for clients receiving services.
- Coordinate with the Payor Acceptance team and Authorizations team to ensure all necessary paperwork is in place for authorization approval.
- Data Management:
- Maintain organized records of all collected documents and communications.
- Update client files and systems with new and updated information as received.
- Customer Service:
- Provide excellent customer service to families and providers, addressing any questions or concerns regarding documentation needs.
- Assist in resolving any issues or delays related to documentation collection.
- Address client concerns as they arise.
- Compliance:
- Track when new documentation is required and when expiration dates are coming up for documentation on file.
- Ensure all documentation complies with organizational policies and regulatory requirements.
- Stay informed of any changes in documentation requirements and communicate updates to relevant parties.
Qualifications:
- Bachelor’s Degree or equivalent prior experience.
- Experience in a healthcare, admissions, social work, or administrative role, preferably within a similar setting.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with a team and independently complete assigned work.
- Proficiency in Microsoft excel, word, teams and outlook
- Experience with electronic health records or similar systems.
