Job Summary
The Case Manager will be responsible for coordinating patient care services in a home health setting, ensuring continuity of care and appropriate discharge planning.
Duties
- Collaborate with healthcare providers to develop and implement patient care plans
- Conduct assessments to determine patient needs and develop individualized care plans
- Monitor patient progress and adjust care plans as needed -Communicating with doctor offices, patients, families. -Following doctor orders to make sure frequency of LVN, PT, OT services are coordinating with orders to be compliant with The Joint Commission.
- Coordinate with insurance providers and other agencies to secure necessary resources for patients
- Provide support and guidance to patients and their families throughout the care process
- Maintain accurate and up-to-date patient records
Skills
- Experience in behavioral health or hospital settings
- Knowledge of discharge planning procedures
- Strong patient service orientation
- Proficiency in understanding medical terminology in history and physical
Job Type: Full-time
Pay: $30.00 - $37.00 per hour
