What are the responsibilities and job description for the Home Health LVN Case Manager position at Choice Health Group?
The Doctor's Choice Home Health is seeking a Full Time LVN Case Manager to join our growing home health team. The LVN will monitor the plan of care to reflect admission database, on-going findings, age-appropriate care, cultural specific needs, and appropriate acuity. It it an on-site position in either Woodland Hills, CA 91367 or Los Angeles, CA 90048. This will be a salaried position.
LVN Case Manager Responsibilities:
- Leads effective development and completion of discharge plans including discharge barriers and patient/family education.
- Discusses with patient, family/significant others about the plan of care.
- Monitors the patient's progress based on their care plan.
- Revises plan on an ongoing basis based on patient condition and evaluation of progress in coordination with the clinical team.
- Answers patient questions about medication purpose, side effects, and administration instructions. LVN Case Manager Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- 1 to 2year minimum clinical case management experience. Community health/hospice or medical/surgical experience is preferred.
- Currently licensed as an LPN/LVN in the State.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
- Works as the liaison between patients, their families and our agency staff.
- Responsible for the coordination of home health services, appropriately reporting the identified needs for home health interdisciplinary group members, including home health aide, OT, PT, MSW, ST, Dietician or Clinical Supervisor
- Responsible for the timely obtainment of visit authorizations from Insurance Providers and subsequent plotting of healthcare staff once authorizations approved.
- Reporting change in patient status requiring change in POC to MD
- Ensuring practitioner Assessments are submitted within 48 hours of completed patient visit
There is also a requirement to be on-call for this position, paid in addition to salary. It is done on weekends and works as a rotation where a specific weekend would be designated to you.
Salary : $70,000 - $80,000
