The Auberle Family Healing Center is an innovative residential treatment program for families struggling with a substance use disorder (SUD). The program is a licensed 3.5 facility. It also provides an array of services including individual, family, and group counseling, psychiatric consultation, medical services, therapeutic activities, parent education, and case management. The program is exclusively available to Allegheny County families referred by child welfare. This innovative program allows families with multiple adults and up to four children to stay together while the addicted person receives treatment.
ESSENTIAL RESPONSIBILITIES:
1) Provide Case Management services to families to ensure all life domains are addressed and positively impacted
2) Serve as liaison to CYF Caseworker and actively participate in Teaming and Conferencing processes
3) Serve as liaison to school districts and educational institutions
4) Participate in educational planning meetings including IEPs
5) Conduct family assessments, develop family service plans, and document family progress in meeting service plan goals
6) Conduct assessments to determine the ongoing needs of the family and the clients
7) Provide individual support to parent(s) as required by service plan and level of client need
8) Facilitate life skills and educational groups
9) Manage and monitor familys CYF and Family Court involvement.
10) Document progress in the clients ECR and in the CYF KIDS system.
11) Begin aftercare planning at the time of admission
12) Transport clients to appointments such as court, to the welfare/WIC office, etc.
13) Establish and maintain positive working relationships with representatives from community supports and resources
14) Make and track timely referral for family services or child specialty services or other services as identified in the service plan
15) Obtain required training hours and topics annually as required by Auberle, county contracts, state regulations and COA standards.
16) Promote the Continuous Quality Improvement philosophy and maintain all Quality Improvement goals and objectives
17) Demonstrate an understanding of and respect for the cultural diversity of consumers, volunteers, staff, and other stakeholders throughout the provision of services
18) Assure compliance with Council on Accreditation (COA) Standards, trauma informed principles, State Regulations, County Contracts, and Agency Policies and Procedures
19) Perform other duties as the Clinical Supervisor, Program Manager, or Facility Director may direct
TYPICAL WORKING CONDITIONS:
1) Requires working flexible hours, including evenings, weekends and holidays
2) Work is performed primarily in the Family Healing Center but may occur in other locations
3) Requires travel for appointments and meetings as assigned
4) Contact with staff and non-staff (e.g., client families, referents, staff from other community agencies).
QUALIFICATIONS:
- A Masters Degree preferred in Social Work, or related field and 2 years of case management experience working with children, youth or families.
- Preference given to individuals with lived experience around substance use or co-occurring disorders
- Working knowledge of community resources, child-serving systems, and other support services; successful case management and record keeping skills
- A demonstrated understanding of the dynamics of child development and the impact of addiction on the family unit; knowledgeable about child management techniques and child appropriate prevention and intervention activities; demonstrated ability to facilitate individual and family meetings and educational groups
- ACT 33/34 and FBI Clearances
- Pre-employment drug screening and ongoing random drug testing
- 21 years of age or older
- Valid PA drivers license and vehicle with registration and insurance.
