SUMMARY
The Case Manager position will assist in the implementation of Youth4ward programming. The Case Manager will foster relationships with local service providers and enroll youth from marginalized communities in established programming. The Case Manager will engage youth with identified needs and facilitate referrals to stabilize social determinants of health.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned:
- Attend, assist in planning, and facilitate events/activities to engage youth in targeted service areas for the Youth4Ward program.
- Facilitate/provide referrals and resources to program participants, including apps and tools to increase awareness of healthy coping methods, including mindfulness and other social determinants of health.
- Assist with research and development of program resources and materials related to topics outlined in the program plan and required by the funder to improve the health and well-being of participants.
- Assist Youth4ward Program Coordinator with collecting all required program paperwork and/or documentation, and ensure that it is entered into approved portals or platforms.
- Assist Youth4ward Program Coordinator with program data collection, including but not limited to: program attendance, demographics, numbers served, communities served, and referrals made.
- Assist Youth4ward Program Coordinator with marketing and recruitment of youth for programming and Youth Action Board activities and events.
- Model healthy, professional, and positive behaviors for program participants, clients, and volunteers.
- Prepare for and attend all scheduled program events in agency locations and designated community service areas.
- Prepare for and participate in all individual, team, department, divisional, agency, and program meetings.
- Complete all Core Curriculum training(s) required for the position.
- Complete all necessary paperwork and documentation, such as timesheets, vouchers, progress notes, incident reports, etc., and submit them in a timely manner.

WORK HOURS*
Willing to work varying hours based on the needs of the program. Hours may include evenings and weekends at any of the AMHW Reporting Centers.
CARE COORDINATION
Individuals in this position may have a leadership or shared role in coordinating care for clients within the program and a specific geographic area.

QUALIFICATIONS REQUIRED

REQUIRED EDUCATION and EXPERIENCE*
Bachelor’s degree in human services or related field required. Previous experience working with at risk/ juvenile justice involved youth preferred.
