PURPOSE
Responsible for efficiently and accurately indexing and quality checking documents in an electronic medical record utilizing the OnBase imaging software interfaced to EPIC. Provides assistance to physicians in the delivery of efficient and effective comprehensive medical care through maintaining organized electronic medical records (EMR). Carries out all duties while maintaining compliance and confidentiality and promoting the mission and philosophy of the organization supported.
Essential Functions
Education and Experience
Required: High school diploma or GED. One (1) or more years of medical records experience or an Associate or Bachelor degree in related field required.
Preferred: Previous medical document imaging experience a plus.

PURPOSE

Provides assistance to physicians in the delivery of efficient and effective comprehensive medical care through maintaining organized electronic medical records (EMR). Carries out all duties while maintaining compliance and confidentiality and promoting the mission and philosophy of the organization supported.
Essential Functions
- Sorts outstanding batches by priority.
- Opens a pending batch to view each individually scanned image and assigns free text and drop-down document identifiers per guidelines to each page.
- Assigns proper medical terminology descriptions and patient identifiers to each documents while indexing.
- Reviews entered fields required for indexing ensuring that the patient name, medical record and other required keywords are correct on every page.
- Verifies the quality of the batch by identifying that pages are in the correct order, the pages or documents are legible, no pages are missing and all pages are oriented and indexed appropriately.
- Takes appropriate action on any scanning image quality errors by utilizing system functionality.
- Maintains indexing standards by memorizing and identifying various documents used throughout entire organization and how they correlate to EMR functionality.
- Indexes each medical record document according to policies and procedures.
- Ability to self-delegate time to ensure productivity standards are met with meticulous attention to detail to ensure document quality standards.
- Communicates errors and process improvements with coworkers and supervisor.
- Meets set productivity standards daily and adheres to departmental error guidelines. .
- Performs duties of HIM Document Scanning Specialist.
- Adheres to all company policies, including but not limited to, OSHA, HIPAA, compliance and Code of Conduct.
- Regular and dependable attendance.
- Follows the core competencies set forth by the Company, which are available for review on CMSweb.
- Demonstrates the ability to change toner, ribbons, paper, and labels for all department equipment.
- Uses computerized systems according to standards.
- Performs other duties as assigned.
Education and Experience
Required: High school diploma or GED. One (1) or more years of medical records experience or an Associate or Bachelor degree in related field required.

- Familiarity with document imaging and workflow systems a plus.
