The Social Worker is responsible for providing psychosocial care to hospice patients and their families regarding economic, social, and emotional needs. He/she will implement and coordinate all social service activities of the hospice program under the direction of the Hospice Medical Director.

Essential Job Functions and Duties:

  • Provide emotional support and counseling to patients and families from intake to discharge through personalized visits.
  • Maintain a minimum Productivity Standard according to Hospice policy. Productive time consists of face-to-face contact with patients and families in the form of direct care activities. Productive time also includes coordination of care activities with families, physicians, and host nursing facility partners, such as: TCMs, at-risk meetings, information presentations, or meet and greets.
  • Perform initial assessment of patients’ level of care needs and develop psychosocial recommendations for the Plan of Care. Present Plan of Care recommendations to interdisciplinary (IDT) team. Develop a comprehensive psychosocial plan based on the IDT assessment.
  • Communicates appropriate psychosocial information to the IDT regarding patient and family needs.
  • Assess family and caregiver needs for respite care.
  • Participate in regularly scheduled IDT meetings to coordinate the care of the patient and family, exchange information and solve problems, and receive staff support and in-service information.
  • Maintain all patient privacy and confidentiality in accordance with Hospice policies and HIPAA requirements.
  • Complete documentation of all patient visits on the clinical record per Hospice policy. Submit all documentation in a timely manner to update and maintain patient hospice medical records.
  • Ensure that all care is provided consistently with respect for patient rights. Report any/all allegations of patient abuse, neglect, and/or misappropriation of property in accordance with Hospice policies.
  • Assess hospice patient/family factors to estimate capacity to cope with terminal illness and death. Provide counseling services to patients and family per Hospice policies.
  • Assist patient/family with financial obligations and assess appropriate resources as needed.
  • Provide support to Hospice Team Members, volunteers, and facility staff as needed.
  • Participate in hospice programs providing staff and volunteer orientation, training, and education.
  • Participate in Hospice’s Quality Assurance/Performance Improvement program as requested.
  • Provide community programs concerning hospice service and related issues of illness and loss.
  • Follow standard precautions and infection control procedures at all times.
  • Attend and participate in all in-service training and meetings as needed.
  • Participate in developing and updating Hospice policies and procedures as requested.
  • Report all accidents or incidents observed. Identify and respond appropriately to emergency situations.
  • Maintain required clinical competency/license as appropriate.
  • Complete other duties as assigned.

Position Qualifications and Entrance Requirements:

  • BSW degree required. MSW degree is desirable.
  • Must be licensed as an LICSW or work under the supervision of an LICSW.
  • Must have at least 1 years’ experience working independently with patients.
  • Must have demonstrable competency with computers (MS Word, MS Excel, etc.)
  • Solid verbal, written, and interpersonal communication skills are required.
  • Experience working independently with minimal supervision is required.
  • Candidates must successfully complete criminal background check and physical prior to completing Orientation.

Note: All positions at our organization are paid positions, including internships.

Job Types: Full-time, Part-time

Pay: From $37.00 per hour

Expected hours: 20 – 40 per week
