Our resident care coordinator will be part of enriching the daily lives of our residents. The fulfilment and satisfaction that comes with helping others surpasses the challenges of this position. Our Resident Care Coordinator will work with residents who require assistance with day-to-day living and overseeing the smooth operations of the community. They will also establish and maintain a positive, respectful, and supportive environment for all residents, co-workers, families and facility guests.
Job Duties;
- Assists with managing floor staff.
- Assist with orients, coaches, counsels, and evaluates nursing team staff.
- Completes job performance evaluations along with the DOW for nursing staff.
- Careful monitoring of healthcare routines for residents. Any irregularities must be immediately reported to the DOW who can stabilize the situation.
- Monitor residents closely according to a plan created by the resident care coordinator, who interacts with other professionals, residents, and their families.
- Performs assessment evaluation for prospective residents
- Works in partnership with the Administrator, DOW, Admissions Coordinator to place residents appropriately within the continuum levels of care
- Will be part of an on-call rotation for nursing department
- Assist with nursing schedule
- Assist with medication orders
- Communicates with outside resources, organizations, hospitals, and doctor officers.
Qualifications/Skills Required:
- Must be able to work some weekends, some holidays and be on-call rotation
- Previous experience supervising or managing staff
- High School Diploma or equivalent is a must
- LPN or CMT experience is required
- Completion of PA Direct Care Test
- Valid Driver’s License
