Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with patients, families, and healthcare teams.
Responsibilities
- Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
- Monitor vital signs and perform routine medical procedures, including IV insertion and specimen collection processing.
- Provide acute care services, including urgent care interventions as needed.
- Assist with discharge planning to ensure smooth transitions from hospital to home.
- Educate patients and families on health management, medication administration, and health coaching techniques.
- Document patient progress and maintain accurate medical records in compliance with healthcare regulations.
- Collaborate with interdisciplinary teams to coordinate patient care and optimize health outcomes.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Experience in acute care settings preferred; experience in ICU or urgent care is a plus.
- Proficient in patient assessment techniques and discharge planning processes.
- Strong skills in vital signs monitoring and IV insertion.
- Excellent communication and interpersonal skills for effective patient education and collaboration.
- Ability to work independently while managing multiple patient cases effectively.
- Previous experience in home health or medical office settings is desirable.
