What are the responsibilities and job description for the RN Hospice position at Avani Healthcare Staffing?
Hospice RN Case Manager HCHB experience required.
- Must have at least 12 months of hospice in home settings experience within the past 3 years. Managing a caseload of patients 45 nursing visits per day communicating concerns with physicians electronic documentation IDT notes attend morning meetings and biweekly IDT meetings communicating concerns with team members report to DCSs. Monday Friday 40 hours per week. No on call or weekends. Area of coverage is Mountain Grove with a 90 minute radius from the office.
