Job Title: Licensed Practical Nurse (LPN) in Long Term Care
Summary: As a compassionate and skilled Licensed Practical Nurse (LPN), you will provide professional nursing care to residents in our Long Term Care facility. Your role involves performing assessments, administering medications safely and accurately, and collaborating with multidisciplinary teams to implement effective care plans.
About the Role:
- Provide professional nursing care to residents, including assessing their needs, developing and implementing care plans, and evaluating resident outcomes.
- Collaborate with registered nurses, physicians, and other healthcare professionals to ensure seamless care coordination and delivery.
- Mentor new staff members and advocate for residents' needs within a compassionate environment.
- Participate in the care planning process and ensure that resident care is implemented consistently and appropriate to the age of those served.
- Communicate effectively with residents, families, and interdisciplinary team members to ensure quality care and support.
- Assist with orientation of new staff and share knowledge with others to promote a culture of learning and growth.
Requirements:
- Licensed Practical Nurse (LPN) - Board of Nursing An active license in the state of practice Upon Hire
- Less than 1 year experience in Long Term Care (preferred)
