Job Summary:
We are seeking a Licensed Practical Nurse (LPN) to join our multidisciplinary team at Avera Health. This role requires a compassionate and skilled individual who can provide professional nursing care to residents in a long-term care setting.
Responsibilities:
- Provides nursing care to residents, including assessments, evaluations, and interventions as directed by the Registered Nurse.
- Administers medications safely and accurately according to facility policy and physician's orders.
- Participates in care planning and ensures quality care is provided to residents.
- Communicates effectively with residents, families, and healthcare team members.
- Maintains accurate records and reports incidents or concerns to supervisors.
- Fosters a positive work environment and promotes a culture of safety and respect.
Requirements:
- Licensed Practical Nurse (LPN) license in good standing in the state of practice.
- Less than 1 year of experience in long-term care preferred.

- Ability to communicate effectively and work collaboratively with others.
