Avera Health is seeking a Therapeutic Care Provider to join our team. As a key member of our rehabilitation services team, you will play a vital role in the coordination and delivery of physical therapy services.
Job Summary
The Therapeutic Care Provider will be responsible for providing direct therapy services to patients using various functional settings and strategies. This includes evaluating assigned patients, developing and implementing individualized treatment plans, and instructing patients and their families on home programs or exercises.
The ideal candidate will have excellent communication skills and be able to work effectively in a team environment. They will also be committed to providing high-quality patient care and adhering to Avera's standards of compassion, hospitality, and stewardship.
Responsibilities:
- Provide direct therapy services to patients using various functional settings and strategies
- Evaluate assigned patients and develop individualized treatment plans
- Implement specific treatment programs for individual patients and patient groups
- Instruct patients and their families on home programs or exercises
Requirements
To perform this job successfully, an individual must be able to perform each essential job function satisfactorily. This includes having visual acuity adequate to perform position duties and the ability to communicate effectively with others.
The individual must also be able to work the hours specified and have an active license in the state of practice. Preferred education, license/certification, or work experience includes being a licensed physical therapist.
