\Key Job Responsibilities
\Coordinate patient flow and manage phone calls and schedules to ensure efficient use of resources.
\Provide administrative support to the care team, including preparing reports and presentations, maintaining accurate records, and addressing patient concerns.
\Collaborate with healthcare professionals to develop and implement patient care plans, while promoting open communication and building trust with patients and their families.
\Ensure compliance with regulatory requirements and organizational policies, staying up-to-date with industry developments and best practices in patient care coordination.
\Contribute to ongoing evaluation and improvement of patient care services, identifying areas for process improvement and developing solutions to optimize patient flow and care coordination.
\Requirements
\High school diploma or equivalent required; three to six months related experience and/or training preferred.
\Excellent communication, interpersonal, and problem-solving skills required.
\Ability to work in a fast-paced environment with multiple priorities and deadlines.
\Proficiency in computer software and technology platforms, including electronic medical records (EMRs) and practice management systems.
\Knowledge of medical terminology, coding, and billing principles preferred.
\Work experience in a healthcare setting, preferably in patient care coordination or a related field.
