Position Summary – MDS Coordinator
As the MDS Coordinator, you will be responsible for the timely and accurate completion of the Minimum Data Set (MDS), Resident Assessment Instrument (RAI), and care plans in accordance with federal and state guidelines, as well as facility policies.
Key Responsibilities
Coordinate and oversee the completion of the MDS/RAI process
Ensure accurate and timely assessments and submissions
Collaborate with the interdisciplinary team to develop comprehensive care plans
Monitor documentation for accuracy and regulatory compliance
Educate and support nursing staff on documentation and MDS processes
Act as a liaison during audits and surveys related to MDS and resident assessments
Qualifications
Current RN or LPN license in the state of New Jersey
MDS 3.0 experience required
Knowledge of Medicare guidelines and reimbursement
Strong analytical and documentation skills
Ability to work collaboratively across departments
Previous experience in a sub-acute or long-term care setting preferred
