POSITION PURPOSE:
The clinical pharmacist plays a crucial role in patient care by working closely in a multidisciplinary team to ensure safe and effective use of medications. The clinical pharmacist works with the retail pharmacist to ensure proper pharmacy management including daily functions of the pharmacy ensuring proper preparation, review, verification, and dispensation of prescribed medication. The clinical pharmacist provides medication management, patient counseling, therapeutic drug monitoring, clinical research, and development of clinical staff pharmacological knowledge. The pharmacist provides a positive customer service experience in accordance with the organization’s mission. The pharmacist will support the retail pharmacy in efficient workflows in receiving and dispensing medications and inventory. The pharmacist may oversee and direct the pharmacy technician staff and provide education to patients to enhance the customer experience.
ESSENTIAL FUNCTIONS:
- Works with multidisciplinary health care team to provide drug information and assist in monitoring patient drug therapies
- Collaborate with physicians, nurse practitioners, and other healthcare providers to develop and monitor treatment plans
- Perform thorough medication reviews to ensure safe and effective use of medications
- Identify and resolve drug-related problems, including drug interactions, side effects, and non-adherence issues
- Provide counseling and education to patients regarding their medications, including proper usage, potential side effects, and lifestyle modifications
- Recommend medication adjustments, dosages, and alternatives based on patient-specific needs.
- Provide one-on-one education for patients with chronic conditions (e.g., diabetes, hypertension, asthma) regarding the proper use of medications
- Educate patients on disease management, adherence strategies, and the importance of preventive care
- Assist in the management of chronic conditions through medication management and follow-up care
- Monitor patients for effectiveness, side effects, and the potential for polypharmacy issues.
- Work to reduce hospital readmissions and improve overall health outcomes.
- Ensure compliance with all regulatory and safety guidelines, including the reporting of adverse drug reactions and medication errors.
- Monitor and evaluate the effectiveness of pharmaceutical services and make recommendations for continuous improvement.
QUALIFICATIONS:
- Bachelor of Science in Pharmacy from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE) required; PharmD preferred
- Valid Oregon state pharmacist license
- BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification will be provided upon hire
- At least two (2) years of professional job experience
- 340B pharmacy knowledge, preferred
- Strong attention to detail, quality focus, and legal compliance
