Position Purpose:
This dual role combines direct patient care as a Nurse Practitioner or Physician Assistant in an outpatient primary care setting with a leadership position overseeing Aviva Health’s Advanced Practice Provider (APP) Residency Program. The clinician provides high-quality, patient-centered care to a diverse, rural population, while also dedicating time to the educational development, support, and supervision of APP residents.

Clinical Responsibilities:
- Provide comprehensive outpatient primary care to underserved populations including low-income, rural, homeless, and otherwise marginalized individuals.
- Perform patient assessment, diagnosis, and treatment of acute and chronic conditions across the lifespan.
- Order and interpret diagnostic tests, prescribe medications, and develop individualized treatment plans.
- Delivery of care consistent with Aviva Health policies, procedures, and quality standards, emphasizing prevention and health promotion.
- Document encounters in a timely and accurate manner in the electronic health record.
- Respond to medical questions and provide patient education regarding medications, health conditions, and preventive care.
- Collaborate with an interdisciplinary care team including medical assistants, nurses, behavioral health, pharmacy, and dental teams.
- Participate in staff meetings, continuing education, and maintain required licensure and certifications.
- Uphold Aviva Health’s mission, vision, and values in all patient and staff interactions.
APP Residency Program Director Responsibilities:
- Provide clinical and educational leadership for the APP Residency Program, ensuring alignment with program goals, learning objectives, and regulatory standards.
- Supervise clinical work of APP residents during precepted sessions, providing mentorship, real-time feedback, and support.
- Be present and available in the POD/clinical area during precepted time to support resident learning and patient care.
- Offer guidance on patient care, documentation, clinical reasoning, interprofessional collaboration, and professionalism.
- Participate in resident recruitment, orientation, scheduling, and formal evaluations.
- Assist in curriculum development, including input on didactics, specialty rotations, and competency assessments.
- Use diverse teaching strategies such as impromptu case discussions, Q&A, reflective feedback, and evidence-based reasoning.
- Model integrated care and encourage residents' involvement in team-based care, meetings, and collaborative practice.
- Participate in program improvement, quality assurance, and occasional grant or reporting-related activities.
Qualifications:
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Oregon.
- DEA certification and BLS required.
- Minimum of 5 years clinical experience; experience in education or precepting strongly preferred.
- Demonstrated commitment to underserved populations and team-based care.
- Strong interpersonal, communication, mentoring, and organizational skills.
- Ability to work independently and collaboratively in a fast-paced environment.
- Knowledge of or interest in postgraduate APP training programs preferred.
