Responsibilities:
- Billing expectations and requirements must be met on a monthly, quarterly, and yearly basis.
- Maintain Wrap Around Plan of Care established with designated supervisor. Actual composition will be determined by the intensity of his/her caseload, the geographical area, and the specific service structure in the region
- Provides assessment and evaluation opportunities to determine the appropriateness and need for supportive services.
- Assists the client's entry into service and monitors that the needed services and benefits are received.
- Provides coordinated, comprehensive development of a treatment plan and monitors that plan as required by established rules.
- May assess client, family, and environments, including transporting persons in corporate and/or personal vehicles and conducting regular home visits.
- Provides crisis coordination for individuals, who are in a personal, social, or medical crisis requiring assistance within 24 hours. The Community Resource Specialist will follow up to assess stabilization or on-going need for crisis intervention.
- Follows State of Montana and AWARE rules, policies, procedures, and guidelines as they apply to the services.
- Prepares reports, applications, and supporting documentation as requested by the supervisor within the timelines established by that supervisor.
- Attends workshops, conferences, and meetings and may participate in committees as agreed upon with the supervisor, presents information to other Community Resource Specialists and AWARE employees, as requested.
- Recognizes and utilizes AWARE’s Core Values, Unconditional Care Philosophy, and wrap around philosophy focused on family engagement.
- Attends and participates in mandatory training, in-services, staff meetings, and committees as assigned and noted in AWARE’s Employee Handbook.
- Must demonstrate – on a consistent basis – the willingness and ability to productively utilize work time in order to meet the needs of the individuals being served as directed, with the understanding that specific fiscal goals (billing) must be met on a monthly basis.
- Records notes, billing, assessments, referrals, and other data entry assignments via AWARE’s Electronic Healthcare Record System - - being sensitive to age, gender, disability, sexual orientation/identity, racial, bi-sexual, transgendered, and ethnic differences of individual served.
- Perform other duties as assigned.
Requirements
Talents, skills, and abilities:
- Ability to, under AWARE’s Unconditional Care Philosophy, develop and implement individual treatment plans with the knowledge of behavioral, clinical, and mental health interventions, skill building techniques, and understanding of the emotional and behavioral needs of assigned clients.
- One-year experience in mental health services or a related field.
- Interpersonal oral and written communications skills to work as a team member, to make observations and to report them accurately, to learn behavioral, clinical, and mental health care methods, and to follow oral and written instructions and have the ability to facilitate meetings.
- A BS/BA Degree in a social work-related field is strongly preferred
- One (1) year of direct work experience in the mental health field. An education equivalency may be considered based on documented employment experience and the PACT service model.
