Location : This will be a Member Facing role, looking for candidates from Hidalgo County or Starr County, TX
SHIFT : Monday- Friday 8 : 00 AM- 5 : 00 PM / Overtime as needed
- Team member needs a solid background in behavioral health and must have a clean, unrestricted license such as; LPC, RN, LMFT, LCSW, etc.

Describe the performance expectations / metrics for this individual and their team :
Position Purpose : Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans / service plans for long-term care members and educates members and their families / caregivers on services and benefits available to meet member needs.
Education / Experience :
Requires a Bachelor's degree and 2 - 4 years of related experience. Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position. Required Skills / Experience : Preferred Skills / Experience : 1. Must hold a LPC, LCSW, LMFT or RN license; unrestricted license required. 1. Bilingual- English / Spanish 2. Driver's license; own means of transportation to complete field visits 2. Experience working in a fast-paced environment and ability to time manage appropriately and work independently. 3. Experience working with children with behavioral health needs / diagnosis 3.
