Job Summary:
The IBH Care Coordinator - Pediatrics is responsible for linking pediatric clients to medical, behavioral, and social services in a preventative and comprehensive manner. This role serves as the liaison between the client, the community, and the full scope of Axis’s services, to include medical care, behavioral health services, dental care and the WIC program. This role will support young clients and their families with the basic medical and social needs to lead healthy lives and to minimize their risks for entering the criminal justice system.
Qualifications:

High school Diploma Required, Associates of Arts or Bachelor Degree preferred.

Two (2) years of experience interacting one on one with patients from diverse backgrounds in healthcare, social services or in a probation setting.

Bilingual Spanish/Hindi or other language prevalent amongst Axis patients preferred, but not required.

Experience working with children and parents.

Knowledge of Tri-Valley/Bay Area health and social service resources.

Strong organizational, administrative and problem-solving skills and ability to adapt, while maintaining a positive attitude.

Ability to work under pressure and complete assignments independently and in a timely manner.

Ability to effectively present information to others, including clients, Axis staff and community partners.

Essential Duties/Responsibilities
-
Accepts warm hand-offs between agencies (or departments internally) and coordinates care to facilitate integration of care for pediatric patients.

Assists IBH Director with program development related projects and administrative support.

Works with IBH Director to implement structures that are designed to enhance patient care through greater integration between outside agencies, Axis medical providers, the consulting psychiatrist and the IBH staff therapists (i.e. obtaining records, sending records, etc.)

Tracks patient attendance on all IBH staff therapist panels, and is partially responsible for enforcing Axis Community Health’s IBH Attendance Policy.

Work closely with therapists who work with children and teens to coordinate patient care and resource needs.

Support the navigation of wrap-around preventative services for clients and families including enrollment in Medi-Cal, CalFresh, housing and other resources.

Assist with implementing universal BH and trauma screenings for pediatric populations, with the goal of early intervention for at-risk clients.

Link clients who have experienced adverse childhood experiences (ACEs) to appropriate follow-up resources both within the health centers and throughout the Alameda County.

Encourage partnership between external entities such as schools, ACBH contracted services, California Children’s Services (CCS), and the Regional Center of the East Bay.

Actively participate in ongoing trainings and workshops that promote continuing professional growth and increase knowledge base around pediatric healthcare service.

Attends and participates in weekly staff meetings as requested; helps IBH Director set up trainings and agenda for staff meetings as necessary.

Tracking of patient referrals internally, externally and other reporting requirements mandated by funding source.

Participates in any other activities deemed appropriate by the IBH Director related to providing additional support to the IBH program.

Works hand in hand with other care coordinators and also care neighborhood professionals in order to ensure all services are being provided in collaboration and are not redundant.
