Referral Coordinator – Full Time – Western slope, CO

Position Summary: The Referral Coordinator oversees patient referrals, ensuring access to care by collaborating with external partners and health systems. They handle patient documentation, communication, and support activities related to patient care. A key aspect of their role is maintaining accurate records and streamlining the referral process to provide timely support for both patients and providers.
Our ideal candidate for our Referral Coordinator position will have their high school diploma, bachelor's preferred. At least 2 years of relevant experience in referral coordination in a healthcare setting.
Salary: Starting Pay Range $17.50-21.05 per hour which considers experience, education and locale if working onsite.
Physical demands: Employee must occasionally lift and/or move 15 pounds up to 50 pounds on an occasional basis. Employees are frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employees must be able to travel between AHS locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. AHS may make reasonable accommodations to enable individuals with disabilities to perform the essential functions.

Required Skills:
- Responsible for the tracking, coordination and communication of Axis patient referrals.
- Responsible for care coordination, care management, referral management, transitions of care, and task administration.
- Prioritizes referrals by their level of urgency and addresses them in a timely manner.
- Reviews details and expectations about the referral with the ordering provider and the patient.
- Assembles information from the patient’s health record to address patients clinical background to address referral needs.
- Ensures complete and accurate patient registration.
- Contacts insurance companies to verify benefits and prior authorization approval requirements are met. Presents necessary medical information such as history, diagnosis and prognosis to insurance companies if necessary to prove medical necessity.
- Acts as the point of contact for patients, specialists and referring entities for questions and reviews details and expectations of referral(s) with the patient.
- Handles requests from third party referral sources and provides relevant information.
- Assists patients in problem solving potential issues related to financial or social barriers (e.g.,

connecting to internal resources for community transportation resources, etc.)
- Communicates patient updates between referring source and patient care teams.
- Scans documents into the patient’s electronic health record so that documentation is available for Axis providers.
- Manges clinics referral fax folders and tasks faxes to appropriate departments or the patient's patient health record.
- Provides referral disposition notices to Axis care team and/or referring party regarding update and status of referral.
- Maintains and updates internal referral smart sheet with information for internal data tracking.
Additional eligibility requirements: Annual Flu immunization, Annual TB screening, CPI training within 90 days of hire, BLS with in 90 days of hire.
