Position Summary: The Wellness Coach supports patient wellness and health integration in outpatient clinics. Wellness coaches are an integral part of a patient’s Care Team and work within multidisciplinary teams and other support staff. This position is responsible for engagement of clinic patients into a variety of healthcare activities through population-based assessments and screenings and tracking patients’ follow-up and progress. Wellness coaches focus on prevention, health education, brief interventions and referral to additional care when indicated.
Our ideal candidate for our Wellness Coach position will have their high school degree (or equivalent). A bachelor’s degree in social work, Psychology, Counseling, or equivalent field is preferred. Two years of volunteer or paid experience in a healthcare or social service setting is also preferred.
Salary: $20.00 (Base Rate $17.50 $2.50 Locale) - $22.05 and considers preferred education and experience.

Physical demands: Employee must occasionally lift and/or move 15 pounds up to 50 pounds on an occasional basis. Employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between Axis locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. - Must have current driver’s license, qualify for commercial vehicle insurance and maintain a driving record that allows insurability with Axis’ insurance agency.

Additional eligibility requirements: Annual Flu immunization, Annual TB screening, CPI training within 90 days of hire, BLS PRO certification required within 90 days of hire.
