Azria Health Winterset is a skilled nursing facility located at 1015 W. Summit. Winterset, IA. provides high quality short-term rehab and long term care to our residents. We are in search for a high energy passionate Social Worker.
Customary Duties
Assists the resident, responsible party, and supportive others in the adjustment and orientation to the nursing home after admission.
Assures timely progress notes on each resident in the facility.
Participates in each resident's total plan of care staffing.
Has a knowledge of the community resources available and use them to benefit and improve the quality of care as applicable for each resident.
Takes advantage of opportunities to inform and make community aware of the nursing home's need and value to the community.
Has an open door policy to all residents, family members, and staff to listen to their problems or needs; do effective counseling or referrals to meet these needs.
Assists other departments within the facility when requested, and when it will benefit the residents and/or help the facility operate smoothly.
Is an advocate for the resident and his/her rights, helping the resident in communicating their needs, wants, and problems to the appropriate departments and agencies.
Plans and organizes family council meetings.
Job Requirements
Social work experience in long term care.
