What are the responsibilities and job description for the Community Resource Coordinator position at CATHOLIC COUNCIL FOR SOCIAL CONCERNS INC?
The Community Resource Coordinator connects community resources and services to adults, adolescents, children and/or families with the goal of promoting individual and family stability and self-sufficiency. Focusing on former Catholic Charities’ Refugee Services clients, this role will also support Food Pantry and walk in clients as needed. Key responsibilities include assessing client needs, making referrals to community resources, and fostering connections with service providers. The Community Resource Coordinator attends continuum of care meetings to build community partnerships and strengthen community roots. Under the direction of the Director of Programs, this position works collaboratively with other program staff to ensure integrated, client-centered services across agency programs.
KEY RESPONSIBILITIES
- Perform assessment of need for current Catholic Charities clients and provide applicable referrals and support.
- Collaborate with surrounding agencies to ensure smooth transitions, providing open communication about current openings, barriers to care, and other client needs.
- Maintain online databases with accurate and up-to-date case records, documenting client interactions and progress. Properly utilize available database tools. Maintain absolute confidentiality of information, written or verbal, according to policy and legal requirements, bringing any concerns to Director of Programs immediately.
- Develop and maintain working relationships with other community-based organizations for purposes of advocacy, education, resource development and public relations.
- Develop, implement, and modify service plans with individuals, families, and ancillary service providers. Monitor and assess clients’ progress towards identified goals and make recommendations.
- Develop and maintain policies and procedures as it pertains to position. Ensure program compliance and adherence to standards.
- Support refugee post-resettlement program services as needed.
- Work with Community Relations Manager to identify and coordinate volunteers who can assist families with essential needs such as transportation and obtaining household items.
- Proactively research grants and assist in identifying funding opportunities to advance program initiatives and sustainability.
- Attend community meetings and events to support program as required.
- Attend regular staff meetings, team meetings, and supervision meetings.
- Provide input and assistance in annual budget plan development.
- Participate in continued training as required.
- Perform other duties as assigned.
- Bachelor’s Degree in Human Services, Social Work, Public Health, or related field; 1-2 years related job experience and/or training; or equivalent combination of education and experience.
- Solid knowledge of human and social service programs and practices, including familiarity with needs and resources in the community.
- Experience working with vulnerable populations preferred.
- Excellent verbal and written communication skills. Ability to effectively present information and respond professionally to inquiries from clients, regulatory agencies, and members of the community.
- Excellent interpersonal skills. Ability to establish and maintain effective working relationships with a wide variety of people having diverse social concerns and problems, while maintaining personal boundaries. Ability to use good judgment in handling various personalities, cultural backgrounds, personal practices or beliefs and different age groups.
- Strong organizational skills with the ability to manage multiple tasks and prioritize effectively.
- Ability to organize and prioritize work assignments while dealing with a constant influx of problems and interruptions.
- Demonstrates initiative and creativity in the identification of solutions to problems.
- Proficiency in Microsoft Office Suite and related software. Ability to learn program specific software applications.
Additional Expectations
- Travel within our service area required on a regular basis. Must have a valid driver’s license and be able to successfully pass Motor Vehicle Record check.
- Ability to pass a criminal background screening, including Child and Dependent Adult Abuse registry check is required. Compliance with Safe Environment training and monthly bulletins.
- Must maintain absolute confidentiality of information, written or verbal, according to policy and legal requirements.
- Attend staff meetings, community meetings, approved seminars, agency trainings/retreats, and professional conferences.
