Responsibilities:

Plan, provide, and coordinate all aspects of direct patient care according to established clinic and departmental policies and procedures, maintaining the highest degree of quality care in a safe environment.

• Provide high-quality patient care and assessment in accordance with established practice standards and clinic/departmental policies in a safe environment.
• Perform patient evaluation, summarize objective clinical findings, and develop an accurate individualized plan of care.
• Ensure that goals are feasible, based upon findings and patient expectations.
• Communicate/collaborate with patient, family, caregivers, significant other, and other members of the healthcare team to promote maximum benefit of care.

Qualifications:

• Master's or Doctorate degree in Physical Therapy.
• Current Physical Therapist license, registration, and/or certification as per state regulations.
