What are the responsibilities and job description for the Community-Based Rehabilitation Services and Case Management position at Balanced Living Behavioral Health?
We are seeking a dedicated and empathetic individual to join our team as a Community-Based Rehabilitation Specialist (CBRS) and Case Manager (CM).
Our agency specializes in working with individuals with disorganization (i.e., hoarding, extreme ADHD) and organizational needs as the basis of our CBRS and CM model. You will provide hands-on assistance, guidance, and advocacy to empower individuals in their homes to achieve their goals and maximize their independence in managing their mental health.
We are looking for a 1099 contracted CBRS/CM to work for us part-time (18-20 hrs) billable hours and possibly move into full-time (35-40 hrs) in time.
We are looking for a person that is responsible, reliable, flexible, willing to learn, organized, hands-on, and understands mental health disorders.
Possible responsibilities include, but not limited to: Direct Rehabilitation Services: Provide a range of rehabilitation services tailored to clients' mental health needs, including psychoeducation, skills training, coping strategies, and behavior management techniques. These areas can include psychological, medical, family, social, financial, occupational, basic living skills, and community/legal issues.
Organize, declutter, and teach coping skills related to those areas. Skill Development: Assist clients in developing and enhancing skills related to managing symptoms, improving social and interpersonal functioning.
Resource Coordination: Connect clients with community resources. Documentation: Maintain accurate and up-to-date documentation of client progress notes and treatment plans. Complete progress notes by Sunday each week in Best Notes. Professional Development: Stay informed about current trends, best practices, and emerging research in community-based mental health rehabilitation, participating in continuing education and professional development activities as required.
Qualifications (requirements):
Bachelor's from an accredited institute- required in Human Services, Early Childhood Development, Family Science, Psychology, Sociology, Applied Behavioral Analysis, or Social Work.
Required to pass a background check with the Idaho Department of Health and Welfare (passed background check expense covered by the company).
Willing to work in clients’ homes and physically help them with organization is required for CBRS.
Background in organizing is helpful or you just enjoy helping people organize their homes.
Case Management can be completed mainly at home.
Previous experience working with individuals with mental health disorders in a rehabilitation or community-based setting is preferred.
Experience using electronic health record systems (will train).
Strong interpersonal skills, with the ability to build rapport with individuals facing mental health challenges in a supportive and empowering manner.
Excellent communication skills, both verbal and written, with the ability to convey information clearly and effectively to clients and colleagues. Knowledge of mental health disorders, treatment modalities, and evidence-based interventions, with the ability to develop and implement individualized plans.
Ability to work independently and as part of a team, collaborating effectively with interdisciplinary colleagues to provide comprehensive support to clients.
Commitment to upholding ethical standards and confidentiality guidelines outlined by professional organizations and regulatory bodies. Extremely responsible and will follow through on scheduling clients every week.
