Examples of Duties
Under close supervision, human services assistant plays a supportive role in providing home and community-based services aimed at preventing unnecessary institutionalization of older adults and individuals with disabilities. This role provides administrative support to the program and will support and assist human services associate, public health nurses and social workers in conducting intake and screening of potential referrals, providing appropriate information and limited case management functions to clients and caregivers, so that they can remain in the least restrictive setting.
- Answers telephone inquiries and interacts with individuals and their families to connect them with public health programs and services
- Under close supervision, determines eligibility for Medicaid programs, creates client profiles and completes intake forms
- Explains program policies, program procedures and financial guidelines to clients
- Initiates procedures to initiate or terminate client participation in our program
- Assists human services associate and other clinical professionals with case management or linkage to services. May make home visits.
- Participates in regular reviews of all client activities with the clinical team and acts on clinician recommendations for intervention.
- Develops and maintains working relationships with clients, families and clinical professionals
- Ensures client confidentiality and maintains individual case records and other required documentation.
- Designs and creates materials and presentations to support clinical team to meet the needs of the clients
Performs other related duties as assigned.

Qualifications
Required Qualifications
Graduation from an accredited college or university with an associate in arts degree in a social science, behavioral science or public health.
Experience in individual, group, or family counseling or in intake and referral work may be substituted on a year-for-year basis up to a maximum of two years for the required education.
Proof of Licenses, Certifications and Education
Applicants are required to submit proof of licenses, certifications and education beyond high school to meet the required and preferred qualifications of the position. Diplomas or transcripts must show the applicant's major field of study. Copies and unofficial transcripts are acceptable.

Salary
Annual Salary review - Step increases may be granted on anniversary date based on satisfactory job performance
