Responsibilities
- Make initial and comprehensive nursing evaluation visits, determine primary focus of care, develop the plan of care within state-specific guidelines, and submit accurate, complete, and timely documentation in collaboration with physician orders.
- Perform patient comprehensive assessments at designated time points and develop the appropriate plan of care.
- Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source.
- Current RN licensure in the state of practice.
