We are currently looking for a Director of Memory Care for The Barclay House of Augusta
Primary Responsibilities of the Director of Memory Care:
- Participates and staffs' quarterly events with the dietary department, marketing department, wellness department and maintenance department.
- Schedules Memory Care Staff.
- Provides memory care med cart audits.
- Coordinate resident care with care partners such as home health and hospice.
- Provides hands on training on memory care for staff.
- Maintains documentation on each resident’s care plan and host care plan meetings with families and care team.
- Ensures that Memory Care Program is implemented for each resident at the time of admission. Follow-up with residents and family members after admission. Assists new residents in adjusting to their unique environment and schedule. Design a plan for new residents to be placed at a dining table with other residents who share similar interests and socialization abilities.
- Participates in outings with memory care residents with the Walk with Me Coordinator including driving the van.
- Manager on Duty Rotation
- Must have a caring heart.
- 2 years of experience working with the senior population, preferably with memory impaired residents and in a long-term care and/or health care setting.
- A high school diploma and/or GED are required. LPN Required.
- CPR Certified
- Working knowledge of MS Word, PowerPoint, and Publisher to develop a calendar.
- Must have and maintain a safe driving record.
