Clinical Pharmacist - Antimicrobial Stewardship (Full-Time Day), Lakewood, NJ

Job Overview:

This position is an Antimicrobial Stewardship Clinical Pharmacist. The pharmacist will ensure appropriate antimicrobial medication utilization. The pharmacist will work collaboratively with infectious diseases specialist physicians on the local antimicrobial stewardship committee data and meeting preparation. The pharmacist will serve as representative for the site on the hospital system Pharmacy Infectious Diseases Collaborative to promote the appropriate use of antimicrobials, improve patient outcomes, and mitigate consequences of antimicrobial resistance.

Qualifications:

Required:

• PharmD
• NJ licensure
• Completion of ASHP accredited PGY-1 pharmacy residency
• Three years of clinical pharmacy experience in hospital if no PGY-1 residency

Preferred:

• Specialized training or experience in antibiotic stewardship and/or PGY-2 infectious diseases residency

Scheduling Requirements:

• Full-Time
• Day Shift
• 40 hours per week

Essential Functions:

• Optimizes clinical outcomes of patients and promote appropriate antimicrobial utilization, while minimizing unintended consequences of antibiotic therapy by using prospective monitoring and guideline implementation.
• Provides accurate, timely, safe, and appropriate medication therapy based on patient factors such as age and needs.
• Responsible for reporting adverse drug reactions, medication errors, and daily activities.
• Actively participates in antimicrobial stewardship and restricted medication programs.
• Actively identifies practice/medication related issues which require evaluation and facilitate quality improvement initiatives or multi-disciplinary education as needed to promote safe practice and advance practice.
• Assists in the development of the clinical skills of the staff pharmacist through educational in-services.
• Precept pharmacy students and/or Residents when applicable
• Perform other duties as directed by department management
