What are the responsibilities and job description for the Patient Navigator Non Clinical position at Barnabas Health?
Job Overview:
The role will assist in efforts to collect information on family social needs,barriers, issues at Childrens Specialized Hospital. The main role of the Family Navigator is to conduct a Social Determinants of Health SDOH screening with patients and their families, and to review patient data and identify appropriate ways to identify and assist families in need. When conducting an SDOH screening, the Family Navigator will effectively engage and establish helping relationships with families while maintaining strict confidentiality of the information collected of patients and their families. The Family Navigator will provide general information as requested, and direct families appropriately, and the Family Navigator will be responsible for communicating social needs, barriers, or issues to a multidisciplinary team at the hospital in an appropriate manner.
Additional responsibilities include monitoring and obtaining accurate information for data collection and assisting with data analysis, and completing other tasks assigned by management.
Remote work is available for this position but the Family Navigator will need to occasionally travel between CSH sites.
