Summary
The Medical Records Clerk is responsible for organizing and maintaining BACHC patient's electronic medical record including personal information, medical history and billing records. Successful candidates will enter medical and financial information into databases, create and implement filing systems, and ensure that the records are being managed according to industry standards. They also confirm with physicians or other medical personnel that the records are complete and correct. The medical clerk has a clear understanding of and commitment to the health center’s mission, vision, and core values.
Essential Functions
1. Collects information from the patients on their medical history, complications, health complaints, test results in order to make a complete electronic medical record of every patient.
2. Compiles the medical records into charts thereby ensuring that all information is present in the patient electronic record.
3. Operates as and when required depending on the shift hours and ensuring all necessary details are being covered in the charts.
4. Checks patient information is recorded accurately and securely in order to process medical records requests.
5. Stays compliant with HIPPA regulations and BACHC’s policies and procedures when providing records in all forms of communications.
6. Ensure documents are scanned accurately and sFax is processed daily.
7. Ensure scans and faxes are picked up from clinical staff and processed daily.
8. Provides excellent customer service to internal and external clients/patients by being responsive to all inquiries in a timely manner.
9. Performs all job duties collaboratively with health center management and exercises good judgment.
10. Processes and responds to all medical records requests within 5 days.
11. Completes forms with complete and accurate patient information.
12. Assists with audits requiring medical records throughout the year.
13. Ensure medical record requests are properly invoiced as applicable, using the usual and customary fee schedule.
14. Performs all job functions in a professional and courteous manner. This includes answering all general phone calls timely.
15. Participates in staff meetings and training sessions as assigned.
16. Receives and sends records for specialty providers and/or groups.
17. Completes telephone encounters from care teams when requested.
18. Works well with department’s additional record clerk and communicates with supervisor in a professional way.
19. Other duties as assigned.
Education and Experience
- High School diploma or GED required.
- Excellent communication skills (verbal and written).
- Excellent customer service and problem-solving skills.
- Excellent organizational skills with the ability to multi-task.
- Basic computer skills (Microsoft Applications, Internet Applications)
- Ability to display adaptability, empathy, patience, and has a positive attitude.
- Fluency in Spanish preferred but not required.

Physical Demands
This is mostly in an office setting; however, this position may require sitting for long periods of time. Walking, bending and reaching may be required as well. While performing the duties of this job, this position is frequently required to do the following:
- Use standard office equipment and access, input, and retrieve information from a computer. Use computer keyboard with manual and finger dexterity and wrist-finger speed sufficient to perform repetitive actions efficiently for extended periods of time.
- Communicate effectively in person or via telephone in a manner which can be understood by those with whom the person is speaking, including a diverse population.

Position Type and Expected Hours of Work
This is a full-time position. Days and hours of work are Monday through Friday with hours of 7:30am to 6:30pm. However, these hours may be change as the clinic standard hours may change.
