Job Summary:
You will operate under the direct supervision of our psychiatrist, collaborating closely with both clinical and non-clinical staff. Your role is pivotal in ensuring safe and effective patient care, while upholding BasePoint's established standards and objectives.
Key Responsibilities:
- Collaborate with the psychiatrist to assist in the development of comprehensive care plans, tailored to individual patients and families, while considering their unique differences and preventive measures.
- Conduct psychiatric evaluations under the supervision of the psychiatrist, ensuring completion within 48 hours of admission by coordinating with support staff.
- Approximately 10 - 12 patient visits per day.
- Work in conjunction with the psychiatrist to maintain a Prescriptive Authority Quality Assurance and Improvement Plan, which includes regular meetings, supervision logs, and consultation documentation.
- Respond to emergencies and crisis situations, consulting with the psychiatrist and working with the treatment team to achieve resolutions.
- Coordinate patient care with ancillary providers, maintain effective communication with referring providers and client family members, and collaborate with support staff for prescription refills and medication changes. Attend weekly care coordination meetings.
- Lead initiatives to enhance patient care and unit operations, promoting outcomes, quality, and patient safety improvements in alignment with organizational objectives.
- Stay updated with industry standards and apply current knowledge to deliver quality healthcare. Contribute to professional practice standards.
- Ensure accurate and complete documentation to support provider billing and meet PHP/IOP level of care requirements.
- Provide relevant management, referrals, counseling, and education to patients, families, and caregivers related to their current care and preventive health measures.
- Utilize data and evidence to develop, implement, and evaluate patient care plans, including ordering and interpreting diagnostic studies and results.
- Act as a clinical resource, interdisciplinary consultant, and patient advocate, leveraging your expertise for the benefit of the team. Pursue opportunities for professional growth.
- Propose and communicate innovative ideas to the team, leading to creative strategies that enhance quality of service and contribute to organizational priorities.
- Serve as a role model and coach to the staff, promoting efficient and effective communication and fostering a collaborative team environment. Encourage active patient and family involvement in their care, actively listening to feedback and supporting organizational goals.
- Actively participate in quality-related initiatives through group processes, workshops, seminars, and staff meetings to enhance clinical and system practice performance.
- Collaborative psychiatrist on site
- Flexibility to work in multiple locations, as needed.
Qualifications:
- Master of Science in Nursing with a focus on Psychiatric/Mental Health from an accredited college or university.
- Required: Two (2) years of behavioral health experience.
- Must be licensed as a Psychiatric Mental Health Nurse Practitioner (PMHNP) by the Texas Board of Nursing.
