What are the responsibilities and job description for the Sr. Admissions Specialist - PRN position at Baton Rouge General Medical Center - Mid City?
Assesses patients within 24 hours of notification and completes assessment within departmental timeframes.
Monitors and assesses issues referring to patients and provides short-term counseling to patients and family members.
Effectively participates as an interdisciplinary team member in developing the plan of care as appropriate to the assigned area. Initiates discharge plans, communicates and collaborates with other healthcare professionals, and updates discharge plans following department policy guidelines. Coordinates community resources and referral sources to achieve optimum results. Conducts group sessions, family & individual as per program schedule or indicated. Attends and participates in treatment team staffing.
SPECIAL SKILL, LICENSE AND KNOWLEDGE REQUIREMENTS
Current Louisiana licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) or LPC.
EXPERIENCE REQUIREMENTS
2 years of experience in Behavioral Health preferred.
Must have a license of CSW/LMSW/LCSW or PLPC/LPC. You cannot work in this position if you have an RSW.
