SUMMARY: Responsible and accountable for creating, implementing, and facilitating patient discharges and transfers from the home setting. The Social Worker collaborates with the patient and caregiver, the patient’s care team and outside entities to ensure safe and timely patient discharges and transfers. Maintains accurate and up-to-date knowledge of all relevant regulations and standards regarding the provision of social services and implementation of treatment and discharge planning.
JOB REQUIREMENTS
Education: Masters degree in an accredited Social Work program.
Licensure: Current Arkansas Social Work license in good standing preferred.
Experience: Minimum 1 year related experience.
Certifications: N/A
Other: Ability to read, write and communicate effectively. Basic computer knowledge required. Driving required.
