Job Title: Patient Care Coordinator
Our healthcare facility is seeking an organized and detail-oriented Patient Care Coordinator to join our team. This candidate will be responsible for the admission and registration of patients, data entry, and handling phone calls to referrals and insurance inquiries. The ideal candidate will have knowledge of community services and resources available to patients, excellent communication skills, be able to multitask, and work well under pressure.
Responsibilities:
- Coordinate the admission and registration process for patients
- Enter patient information accurately into the database
- Answer phone calls and respond to inquiries from referrals and insurance companies
- Verify insurance coverage and obtain necessary authorizations
- Schedule appointments and follow up with patients as needed
- Maintain patient confidentiality and adhere to HIPAA regulations
- Maintain organizational system and filing for Supervisors
- Provide support for the patients
- Coordinate communication between patients, family members and medical staff

Requirements:
High school diploma or equivalent- Must have experience as a Medical Intake coordinator/scheduler
- Case Management experience
- Excellent communication and strong customer service skills
- Strong attention to detail and ability to multitask
- Strong in data entry and computer skills
- Knowledge of medical terminology and insurance verification preferred
- Bi-Lingual a plus
