POSITION SUMMARY :

Under the direction of the supervisor case manager, this position is to coordinate outside specialty appointments for patients, prior authorizations, and following up on referrals. The referral coordinator will promote continuity of care, provide patient education, and provide health maintenance to clinic patients. The referral coordinator will triage assist with patient care on a as needed basis.

ESSENTIAL FUNCTIONS :

  • Expediting patient referrals to specialty
  • Completing referral requests as needed from patients' primary care
  • Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.
  • Provide appropriate clinical information to specialist as needed / requested.
  • Prioritizes referrals by their urgency and addresses them in a timely
  • Assembles patient information, and ensures complete demographic, insurance information, and appropriate / pertaining clinical information is sent to referred
  • Contacts insurance companies to ensure prior approval requirements are Presents necessary medical information such as, history, diagnosis and prognosis to insurance companies if deemed necessary to prove medical necessity of services.
  • Reviews details and expectations about the referral with both ordering providers and patients. Requests new referrals to be ordered when applicable.
  • Assists the medical provider as a Licensed Practical Nurse or Certified Medical Assistant in the daily operations of the outpatient medical clinic.
  • Demonstrates positive communication through friendly, pleasant and professional verbal skills.
  • Demonstrates the ability to meet the needs of the patient (both internal and external customers) in a caring, competent manner.
  • Meet patient needs through telephone nursing and non appointed urgent care visits. Telephone nursing provides information to patients to allow them to gain access to appropriate care and making informed decisions, while encouraging self-care and preventative measures for maintaining health.
  • Administer oral, rectal, subcutaneous, intra-muscular and intra-venous medications, fluids and immunizations as prescribed by provider or by standing order.
  • Performs tests such as ECGs, spirometry, pulse oximetry, and vision screening.
  • Direct patient care; collaborate with data collection, documentation in the electronic health record, and patient / family education.
  • Communicates in order to collaborate, coordinate, and maintain continuity of quality of care with all interdisciplinary team members.
  • Expedite patient plan of care, and education; problem solving with other clinic
  • Actively participates in daily huddle with the clinical support team and provider to review the daily schedule and enhance patient flow.
  • Must attend all mandatory or departmental training offered by the Bay Mills Health Center and / or BMIC Human Resource Department.
  • Other duties may be assigned within the scope and complexity of this position's essential functions.

Requirements

PHYSICAL REQUIREMENTS :

While performing the duties of this job, the employee is regularly required to sit, stand, and walk. The employee's work is completed from the medical clinic area and involves direct, daily contact with sick patients. The employee may come in contact with communicable diseases and appropriate precautions must be taken to prevent the spread of these illnesses or diseases. Some diseases have the potential to be life threatening as in the case of some blood borne pathogens and specific protective measures must be followed at all times. The employee may be required to lift patients or other strenuous physical activity when rendering first aid or while providing patient care. Employee completes their work under normal office conditions, temperatures, and is environmentally controlled the majority of the time.

POSITION REQUIREMENTS :

  • Active Michigan State License for Licensed Practical Nurse or CMA certification required.
  • Must have at least two years of experience in a clinical outpatient setting
  • Experience in working with and / or knowledge of Native American
  • Must possess a personable, client friendly attitude along with organizational ability to handle multiple tasks simultaneously.
  • Must successfully pass all background checks and drug
  • Must have excellent past work history and excellent attendance record as demonstrated through employment
  • To perform this position successfully, an individual must be able to satisfactorily perform each function listed under the essential functions and physical demands categories of this position description.
  • WORK SCHEDULE :

    Full-time; 52 weeks per year. In addition to regular business hours, the person in this position must be flexible around the needs of the employer, which may include occasional evening and / or weekend hours or occasional travel for employer business.
